Intake Related
After eligibility has been established by calling our intake coordinator, you will receive instructions on how to complete intake documentation. Please have copies of DD214, picture ID, utility bill, and marriage certificate (if spouse of veteran) readily available.

Every Six Sessions
Once a therapist is assigned, she or he will provide instructions for assessment completion. Assessments are administered every six weeks in accordance with grant and program requirements. Below are examples.
